a. Hard Copy of Haj Application Form, duly signed by Hajis and pasting of Photograph.

b. Original International Passport.

c. Passport Size Photo Graph

d. Xerox Copy of International Passport.

e. Xerox Copy of Address Proof mentioned in Haj Application Form.

f. Original Payment Receipt of Advance Haj Amount of Rs. 81,000/- each.

g. Medical Screening and Fitness Certificate (as per format available in the Guidelines

h. Bank Details of Cover Head.
